  • Compassion: Providing thoughtful, patient support that recognises your emotional and social wellbeing.

  • Equality and inclusion: Respecting different backgrounds, beliefs and communication needs, and making information accessible.

  • Partnership and improvement: Listening to feedback, learning from concerns and involving you in improving your care.

Care built around you

We recognise that every person’s needs are different. Our planned services include personal care, help with meals and drinks, mobility support, companionship, agreed household tasks, community access and respite for family carers. Medicines support will be provided only where individually assessed, agreed in the care plan and delivered by appropriately trained and competent staff.

We aim to support adults aged 18 and over, including older people, people living with disabilities or long-term health conditions, and those needing support following hospital discharge. Each care arrangement will depend on assessed needs, our staff’s skills and our ability to provide safe, reliable support.

You will be involved in planning and reviewing your care. With your agreement, we will work alongside your family, representatives and relevant health and social care professionals to maintain continuity and respond when your needs change.

Safeguarding

We have a zero-tolerance approach to abuse, neglect or poor practice. All staff are trained to recognise, respond to safeguarding concerns and understand their duty to protect Adults at Risk.

Care Planning & Reviews

Every service user receives a personalised care plan based on assessment of needs, risks, preferences and expected outcome. Care plans are reviewed regularly and updated to reflect changing needs or circumstances.

Continuous Improvement

We actively monitor service delivery, gather feedback and review outcomes to ensure continuous improvement and accountability.
